Liability Waiver

I am voluntarily participating in the activity. I am aware of the risks which may include but are not limited to, physical or emotional injury, pain or vision impairment. I assume all relative risks both known and unknown to me of my participation of this activity.

I also acknowledge that all sterling silver & gold metals will tarnish over time, due to coming into contact with (but not limited to); perfumes, skin creams, hairsprays, tanning products, perspiration, acidic skin pH, chlorine, sea water, hot and humid conditions etc.

I understand that these bracelets/anklets are permanently welded on, and can not be removed whilst exposed to the aforementioned conditions. *Aftercare advice will be provided during the appointment.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
